Between August 26 and September 2, 2025, a computer malfunction at the California Department of General Services (DGS) Office of State Publishing (OSP) caused it to misprint a batch of mailers related to active workers’ compensation cases handled by the Department of Industrial Relations’ Division of Workers’ Compensation. These mailers, called datamailers, are normally auto-populated from a case data file, but the malfunction pulled information from the wrong fields. As a result, some mailers printed a person’s Social Security number in a field meant to show the date of a workplace injury — without labeling it as a Social Security number at all. This appears to be an accidental disclosure caused by a system error rather than any intentional hacking or theft.
